Treatment Consultation Forms and Contact Details
Treatment consultation forms are completely confidential and for therapist’s information only. No data is stored on a computer. Forms are kept in a secure home office for seven years following last appointment (condition of insurance). Forms may be accessed by client at any time.
Mobile numbers may be stored on therapist’s phone for arranging and confirming appointments only. Email addresses may also be stored on the therapist’s phone/email service.

Contact Forms: When you send a message through the contact form, the information is sent by email to angela@angelamacaulaytherapies.uk and stored in the email service. This information is only used to contact you.

You can also request that we erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.
